Accreditation for Pharmacists
The American Society of Health-System Pharmacists is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This program provides 1.5 hours (0.15 CEUs) of continuing education credit (program number 204-000-09-408-H05P).
Methods and Format
This is an online activity consisting of audio and slides for three presentations, a post-test, and an activity evaluation tool. Participants must view all presentations, take the activity post-test, and complete the course evaluation to receive continuing education credit. A minimum score of 70% is required on the test for credit to be awarded, and participants may print their official statements of continuing education credit immediately. The estimated time to complete this activity is 1.5 hours. This activity is provided free of charge.
Target Audience
This activity is beneficial for pharmacists practicing or with an interest in the areas of cardiology and anticoagulation therapy. Pharmacists, including both clinicians and administrators, who are involved in ensuring compliance with Joint Commission standards or the development of anticoagulation guidelines and protocols would especially benefit from this program.

At the conclusion of this educational activity, participants should be able to
- List five of the most common compliance issues with the Joint Commission National Patient Safety Goal (NPSG) on Anticoagulation Management based on early 2009 experience and one method to correct each of these issues successfully.
- Describe the three requirements for a standardized protocol for anticoagulation therapy as required by the NPSG.
- Identify at least three challenges pharmacists face when managing anticoagulation therapy, as well as one potential solution for each of these challenges.
- Summarize key anticoagulation management issues not related to the NPSG.
