Howard I. Cohen R.Ph., M.S., FASHP, is Corporate Director of Pharmacy Services for Universal Health Services, Inc., one of the nation’s largest healthcare management
companies, operating acute care hospitals, behavioral health facilities, and ambulatory centers nationwide. He previously served as Director of the Department of Pharmacy
Services at Thomas Jefferson University Hospitals, Inc., Philadelphia, Pennsylvania. He is also adjunct faculty at Temple University College of Pharmacy in Philadelphia,
Pennsylvania.
Mr. Cohen earned his Bachelor of Science degree in pharmacy from the University of Kansas School of Pharmacy, Lawrence, Kansas and his Master of Science degree in
hospital pharmacy administration from Temple University School of Pharmacy. He completed a fellowship at the Wharton School of Business - Leonard Davis Institute at
the University of Pennsylvania.
A fellow of the American Society of Health-System Pharmacists, Mr. Cohen is also a member of the American Pharmaceutical Association and several state and local
professional societies. Mr. Cohen is the Editorial Advisor for the Pharmacy Net Guide and is an experienced speaker and advisor on various pharmacy and hospital decision making topics.

Darren W. Grabe, Pharm.D. is Associate Professor of Pharmacy Practice at the Albany College of Pharmacy and Health Services, Albany, New York. He is an alumnus of the
Albany College of Pharmacy where he also completed a Nephrology/Internal Medicine residency and a Nephrology Fellowship. Following completion of his fellowship, Dr.
Grabe accepted a position with Texas Tech University School of Pharmacy, Abilene, Texas where he spent two years. He returned to the Albany College of Pharmacy in March 2000.
Dr. Grabe’s area of interest is nephrology. He focuses on patients with chronic kidney disease Stages 1-5 with primary attention to those not yet on dialysis. His major
research objective is to characterize and optimize pharmacotherapy in this specific population in order to decrease hospital admissions and decrease potential and actual
drug related problems. In addition, Dr. Grabe has a keen interest in oxidative stress as it pertains to medications and conditions.
Dr. Grabe has served as the Albany Regional Facilitator for the New York State Chapter of the American College of Clinical Pharmacy. He currently serves as the chair of the
Nephrology Practice Research Network of the American College of Clinical Pharmacy and as co-chair of the medical advisory board for the Northeastern New York (NENY)
chapter of the National Kidney Foundation (NKF). He is a founding member of the Albany Nephrology Pharmacy (ANephRx) group.

Marlo A. Blazer, Pharm.D. is an outpatient oncology pharmacist at the Arthur G. James Cancer Hospital at The Ohio State University Medical Center, Columbus, Ohio. For the past four years, her practice area has been centered in the James Care Outpatient Clinics. Before that she practiced as a home infusion pharmacist with Columbus Children’s Hospital in Columbus, Ohio. Dr. Blazer graduated from The Ohio State University College of Pharmacy with a Bachelor of Science degree in pharmacy in 1998 and with a Doctor of Pharmacy degree in 2007.
Dr. Blazer predominantly works with patients who have solid-tumor malignancies. She is actively involved in initiating supportive care measures such as management of
chemotherapy-induced nausea and vomiting, anemia, pain, and neutropenia, and in the monitoring of patients undergoing chemotherapy. As a member of the Hematology/Oncology Pharmacy and Therapeutics Subcommittee, Dr. Blazer is involved in guidelines development relating to the provision of care for oncology patients within her setting. In addition, she is a lecturer at The Ohio State University College of Pharmacy in the areas of Colon Cancer and Lung Cancer. Dr. Blazer is a member of the Hematology/Oncology Pharmacy Association.
